Request for Public Records

Overview

Requests for public records are fulfilled under the guidelines of the Illinois Freedom of Information Act (5 ILCS 140). Dana McCarthy is the appointed Freedom of Information Act (FOIA) Officer for the Village of Long Grove. The FOIA Officer is located in the Village Hall, 3110 Old McHenry Road Long Grove, Il 60047.

 

 

 

How to Request a Record

 

Complete our Online Public Records Request Form

OR

Submit in writing to the attention of the Freedom of Information Act Officer to 3110 Old McHenry Road Long Grove, Illinois 60047. Requestors may use the printable Village of Long Grove Request for Public Records form attached below or may submit their own written request for records. Requestors choosing not to use the provided form should provide their full name, email or street address, phone number, a detailed account of the documents they are requesting and whether or not they are requesting the documents for commercial use.

Written requests may be submitted in person at Village Hall, mailed or e-mailed to the attention of the Freedom of Information Act Officer, or faxed to 847-634-9408. Requestors will be contacted within the time allotted by law with a response to their request; at that time the requestor will be informed of any fees associated with their request (The Village will disclose the public records requested on the Request Form within 21 Business Days after the receipt of this Request Form for all requests made for commercial purposes, and within five Business Days for all other requests unless the applicable response period is extended as provided by law or the request is denied).

Fees

Unless you have requested and received a waiver (subsection B within the Request for Information form), the following are the fees for public records(copied or certified): 

Copies
8 ½ x 11 or 8 ½ x 14 Black & White
First 50 pages - Free
Additional pages - $.015 per side

Other Types of records with set fees - 
Certification - $1.00 per record, plus copy cost

Certified Documents

For an additional charge, documents may be certified. Certified documents have a signed and sealed letter from the Village Clerk stating that the attached documents are true and correct copies of the original documents on file with the Village of Long Grove. The Clerk’s Office advises those needing public records for legal proceedings to have their documents certified.

Please note that the requestor will be notified if any records they have requested need to be sent out for reproduction/printing. The requestor will be charged at cost for said reproductions/printing jobs.
