(a) any recent marketing materials utilized to promote your service or event; (b) W9 Form; (c) State of Illinois licenses and registrations; (d) non-profit organizations submit list of five highest paid employees and the Board of Directors; and (e) non-profit organizations submit documentation of date the organizations was established.
